Updated Process for Westfield Appointments & System Access

In an effort to continually improve our information security, we have enhanced the process for producers and non-producers to obtain access to Westfield systems, and have updated our producer appointment form. In addition, we have also updated our authorized administrator form.

The updated versions of the forms, HM-765 Request for System Access and Producer Appointment Form and Agent’s Web Passport Authorized Administrator(s) Confirmation Form are included below. We request you begin using the updated forms and new process immediately. Previous versions of the forms will no longer be accepted.

Overview of Changes

Critical Update
New process for requesting system access and/or producer appointment. This form needs to be used for new user access requests, nothing needs to be done for current users.

Key Change:
To align with increased information security practices, the self-administration tool will be no longer be used by Authorized Administrators to create User ID’s for WesCom Access.

HM 765 Form

Non-producer: Section 1 of the form will need to be completed and submitted for ALL users requesting system access.

Producer: A producer requesting appointment with Westfield, regardless of system access needs, will complete ALL sections of the form.

Definitions have been added to clarify the difference between an agency employee (W-2) and Independent contractor

Authorized Agency Administrator Form
Form has been redesigned in conjunction with our upcoming single sign-on conversion initiative.

 The updated HM 765 Request for System Access and Producer Appointment and Authorized Agency Administrator can now be found on AWP.
