Driver's Accident Report
Prepare your drivers for the unfortunate event of an accident by printing a copy of our Driver’s Accident Report. This report is designed to assist drivers in collecting appropriate information in the case of a vehicle accident to support your company and Westfield in better managing and evaluating claims.

For convenience, print double sided so both pages are on one sheet of paper. Fold the sheet in half lengthwise to create a report form that can be easily stored in the glove box or other area of the vehicle.
